What you should bring to the Funeral Arrangement Conference:
- The deceased’s social insurance number. We need the number in order to provide death notification to Service Canada (regarding Canada Pension / Old Age Security).
- The deceased's Saskatchewan health card number. We need the number in order to register the death with the province of Saskatchewan.
- The deceased's birth certificate, as well as vital statistics information, such as the deceased's full name, current residence (street address or land location), parents' names and places of birth, and the occupation that was held for the majority of the deceased's life.
- Cemetery information, if a plot has already been purchased.
- A complete set of clothing for the deceased, including underclothing (shoes are not necessary, but will be put on if brought in) and glasses, if worn.
- A rough copy of the death notice or life history including all surviving family members, their spouses and places of residence,
- A good quality, recent photograph of the deceased. This can also be provided to us by electronic means (e-mail or memory device).
